Preparing for your first appointment:

*Before your appointment please make sure you eat breakfast or lunch.

*Please do not wear perfume or cologne in our office.

*While masks are no longer required, please respect those who choose to wear one.

*Please bring with you a medical history, written in your own words: from birth until now, including childhood diseases, surgeries, fractures, concussions, motor vehicle accidents, other physical or emotional traumas and medications. Also bring any x-rays or other tests pertaining to the reason you are requesting acupuncture.

*Please bring your insurance information.

*You will be responsible to make your co-payment and/or the portion that your insurance does not cover on the day of your first treatment. We accept check, cash or credit/debit and HSA cards.
